Vaccines Required: Withing the last 12 months

Dogs Rabies DA2PP Bordetella (Kennel Cough)

Cats Rabies FVRCP

All Pets are required to be Flea & Tick Free

All Pets must have negative fecal check within the last 12 months 

If you have not brought verification or we can not confirm current vaccination, Petworks will vaccinate your pet accordingly

Should any parasites be found, Petworks will use the appropriate treatment.

  
  

If you have more than one pet, would you like them to be kenneled together? If answering yes to this question, you also understand and assume full responsibility for the possible risk associated with multiple animals that are boarded in the same kennel. 

On Occasion. a pet may become ill while boarding. The most common ailments are diarrhea, vomiting, and lack of appetite. Do we have your permission to treat these ailments for your pet? 

If a serious medical emergence arises while your pet is boarding with us, we will do our best to contact you at the number (s) you provided. However, in the event we are unable to contact you, do we have your permission to treat your pet?
